Could someone do me a solid and post this 2 files out of your module folder?

Why not delete your entire voice installation and re-install? Something is obviously wrong with your current installation. My two cents.

I would assume I screwed it up, if I was alone. I’m not.
Theres gotta be some dependency conflict with the script.
